Box 426008, Del Rio, TX 78842-6008. If a computer or refund form is unavailable, on a 3”x 5” card clearly print your full name, your street address, city, state, primary phone number and zip code (no P.O. Boxes), a brief description of why you are not completely satisfied with our product and mail the 3” x 5” card in a stamped, business-size envelope along with your original receipt.
