One rebate form for Listerine Smart Rinse is available. Product must be purchased by 12/31/08. The full purchase price up to $3.89 will be refunded by mail. If you are interested in the form, please inquire thru the comments section. I will select one reader to receive the rebate form and then mail the rebate form to the reader free of charge. The rebate must be postmarked to the rebate processor by 1/15/09.
